Q: I have filled the substitute W9 out and sent it back to you, why have you sent it again? A: If you have sent us a form before and have received another form, the three most likely scenarios are 1) You filled in a different address than what we have for you in our AP system (i.e. corporate address rather than physical location; or PO Box rather than physical address, vice... versa) or 2) You have moved and your address has changed. If this is the case, you need to contact your Intel buyer and have them submit the change request. 3) You did not fill in your name the same as what we have for you in the system. Please keep in mind that Intel should have you listed by how you are requesting on your invoice. Q: What are your requirements for completing a W9? A: We need the name and address to match exactly with what information we have for your company in our records. Tax-id number must be field filled in. Q: We are no longer doing business with you, or don’t want to be in your AP system. What can we do to remove ourselves?
