CAUSE: You opened a Signal Tap File (.stp) of which the content was automatically stripped by the Quartus Prime software after a compilation. The file name is restored back to its original name. However, the file with the new name is already open in the Quartus Prime software.

ACTION: Close the open file. Then, open this file again.
