Problem:  Cyclone II PLL clock output is non-functional with certain
	  phase shift settings.


Solution: The problem is that the PLL VCO setting at 225 degress was
	  wrong, and the output counter phase was ignored when setting
	  the VCO phase. This patch fixes the problem, and the fix
	  applies to all the Stratix II and Cyclone II devices.


Caution - You must either have previously installed the Quartus II 
          software version 5.0 SP1 or must install the Quartus II 
          software version 5.0 SP1 before installing this patch. 
          Otherwise, the patch will not be installed correctly and 
          the Quartus II software will not run properly.
