If you are accessing the site for the first time on the Internet, you will be asked to create a User ID and password to login. Your User ID is a personal identification that you set up. The Health for Life Centers are full-service primary care facilities designed to provide you and your family a unique, personalized health care experience. Each center is staffed with experienced physicians, nurse practitioners, registered nurses, physical therapists and other support medical staff.
